Completing the Application for Degree |

As a Psychology Major, follow these steps to complete your Application for Degree: 1. Print your name on the application as you would like it to appear on your diploma. 2. Indicate your name as it appears on UAB records. 3. Print your student number. 4. Type of Degree: As a Psychology major, you should circle "B.S." (for Bachelor of Science!). 5. Indicate the semester and year in which you intend to graduate (e.g. Fall 2005). 6. Under Required Courses, there are two columns: First column: List the courses for which you are currently enrolled which count toward your Psychology major, your minor, or the core curriculum. Second column: List the courses in which you will enroll for your final term, that count toward your major, your minor, or the core curriculum. 7. Under Elective Courses, there are also two columns: First column: List all electives in which you are currently enrolled. Second column: List electives in which you will be enrolled during your final term. 8. Under Additional Courses: Incompletes, Transfer Credits, etc., list courses in which you have an "Incomplete" and/or courses that have not yet been transferred but need to be included when your degree requirements are calculated. 9. If you are a University Honors student, you will need to obtain the signature of your University Honors Program advisor in the section entitled Honors Information. 10. Major: Print "Psychology" on Line 1 of this section, with the following exceptions: Students completing the Honors Program in Psychology should print "Psychology Honors" on Line 1 instead. If you will receive a double major, your primary major goes on Line 1 and your secondary major goes on Line 2. 11. Minor and Concentration: Print your minor(s) and/or concentration on these lines. 12. Leave the lines for Major Advisor and Minor Advisor blank. These lines are for the signatures of the advisors in those departments and will be obtained once you have submitted your Application for Degree. 13. Complete the information at the bottom of the form, including your signature. It is important that this information is current and correct. The address you list will be the one used when mailing your diploma and should be the same address that is listed for you on the University computer system. The phone numbers are used in case there are questions regarding your Application. If you need to update this information, please visit the Student ACCESS site or the Office of Registration and Academic Records in the Hill University Center. 14. The Hometown you list will be printed beside your name in the commencement program. 15. Once you have completed the Application for Degree, you should turn it in to the Registrar's Office. A $50.00 non-refundable graduation fee will be charged to your account upon submission. |
